Patient Categorization

Three types of patients are channeled to our providers. Below is a further explanation of each, including some tips on how to best manage each category.

Tier 1 Patients – Physicians and their families
These patients are physicians and family members of physicians.

  • Tip 1: Tier 1 Patients are designated on their ID cards by a star and red stripe across the bottom of the card. For an example of the ID Card: click here (pdf)
  • Tip 2: Tier 1 Patients have no Co-Pay. Please DO NOT ask for a Co-Pay.
  • Tip 3: Ensure the claim is verified.

Tier 2 Patients – Staff members of physicians and their families
These patients are always staff members of physicians and/or family members of the staff members of physicians.

  • Tip 1: Tier 2 Patients DO have a co-pay
  • Tip 2: Tier 2 Patients are designated on their ID cards by a blue circle and blue stripe across the bottom of the card. For an example of the ID Card: click here (pdf)

Tier 3 Patients – Employees of large employers in your area
These are commercial patients of large employer groups

  • Tip 1: Please collect the Co-Pay on the front of their ID Card.
  • Tip 2: If you have any questions, notify the Plan Administrator. The contact number should be located on the card.
