About Your ID Card

It is suggested that you carry your ID card on you at all times. Upon your visit to your provider, it is recommended that you always present your ID Card to ensure that you will receive optimal benefits at the lowest cost advantages to you.

Interpreting the logos on your card

benefit trust

This logo on the ID Card indicates that you are covered by PHA Group Benefit Trust. Individuals with this logo on the card are either a physician, physician family member, physician staff or physician staff member.

CMFC and PHN

This logo indicates that you can see any provider in the Physicians Health Network.

Symbols on your card

If there is a star or circle symbol in the right hand corner of your card, that means that your employer has employee benefits coverage through Physicians Health Group Benefit Trust.

star

The star indicates that you are a preferred physician member or family member of a preferred physician member that has health coverage under the physicians health benefit trust

circle

The circle indicates that you are a physician staff member or family member of a physician staff member that has health coverage under the physicians health benefit trust.

No Symbols on your Card

If you DO NOT have a star or circle symbol on your ID Card, but you see the Physicians Health Network Logo, that means your employer uses another organization for your benefits, but subscribes to Physicians Health Network. This means that you can see any provider in the Physicians Health Network. For questions about your benefits, you should contact your HR representative.
